Client Authorization & Terms and Conditions

Adore Home Care Nursing Services

Welcome to Adore Home Care Nursing Services. By availing our services, the Client (patient or authorized guardian) agrees to the following terms and conditions regarding medical care, caregiver support, equipment rental/sales, and virtual health services.

1. Consent for Medical and Support Services

  • The Client authorizes Adore to provide medical and support services, including doctor consultations, nursing care, physiotherapy, caregiving, and telehealth sessions. Services may be delivered at home, hospital, or other designated settings.
  • The Client further agrees to allow Adore to administer prescribed medications and treatments as instructed by the treating physician. Potential side effects and risks will be explained by qualified professionals. Adore will not be liable for any unexpected reactions, complications, or incidents arising from treatment.

2. Scope of Caregiver Services

  • Duties performed by the caregiver will be limited to the agreed scope of work.
  • All roles, expectations, and responsibilities must be clearly communicated before duty commencement.

3. Safety & Valuables

  • Clients are advised not to keep valuables, cash, or sensitive documents in accessible areas.
  • The client must ensure a safe and secure working environment for caregivers.

4. Professional Conduct & Confidentiality

  • Adore caregivers are instructed to maintain client confidentiality and professional behavior at all times.
  • Any breach of conduct or confidentiality should be reported to Adore immediately.
  • Clients are expected to treat staff with *respect and dignity*. Rude or inappropriate behavior will not be tolerated.

5. Payment Terms

  • Advance payment is mandatory for all services and packages, including rentals and medical equipment sales.
  • All payments must be made *only to Adore Home Care Nursing Services. Clients should *not offer cash, gifts, or tips directly to caregivers.

  • Shipping, delivery, or installation charges for equipment or services are to be borne by the client.

  • Any emergency deposit collected will be adjusted or refunded post-service after accounting for any incidental charges.

  • Applicable taxes (GST) will be levied on all bills.

6. Medical Information Consent

  • The Client allows Adore to access and share medical data (e.g., lab reports, consultation notes, prescriptions, imaging) with doctors or healthcare professionals involved in their treatment. All data will remain confidential and used solely for care coordination.

7. Emergency Protocols

  • In case of medical emergencies, Adore is authorized to initiate appropriate action, including hospitalization or external medical intervention.
  • The Client is responsible for organizing transportation and bearing all related costs.
  • If the caregiver experiences any health issue while on duty, the Client must notify Adore immediately.

8. Caregiver Leave & Replacement

  • Caregivers are entitled to two leaves per month.
  • In case of absence or emergencies, a replacement will be arranged, subject to availability.

9. Minimum Rest Requirement

  • To ensure proper service, caregivers must receive at least 8 hours of rest per 24-hour period, either during the day or night, depending on the shift schedule.

10. Food & Hygiene for Caregivers

  • Clients are expected to provide clean, fresh, and hygienic food to caregivers during their stay. Please avoid offering stale, leftover, or spoiled food

11. Telehealth Services

  • Virtual consultations may be conducted via video or voice, depending on connectivity. The Client consents to the recording and secure storage of consultation records for care continuity. All digital data will be kept confidential.

12. Consent by Legal Guardian

  • If the patient is a minor, elderly dependent, or differently-abled individual, the signatory confirms they are the legal guardian or authorized representative and have the right to provide consent.

13. Insurance & Coverage

  • The Client understands that Adore’s services and equipment may not be covered under all insurance plans. Unless confirmed in writing by the insurer, all expenses are to be borne by the Client.

14. Equipment Rental & Sales Terms

  • Equipment is delivered in working condition. Rentals are charged monthly or as per contract.

  • No refund will be provided for early return of rental equipment *mid-month*.

  • All shipping and return charges are to be borne by the client.

  • Any damage, misuse, or loss of equipment will be chargeable.

  •  Equipment sold is non-returnable unless a manufacturer defect is reported immediately upon delivery.

15. Cancellation & Termination

  • Services may be canceled or terminated by either party with a minimum 7-day notice.

  •  Refunds (if applicable) will be processed within 7–10 business days after submission of all necessary documentation and internal review.

16. Amendments & Consent Validity

  • This consent remains valid until withdrawn in writing by the Client. Adore reserves the right to revise these terms at any time. Updated policies will be shared via email or on our official website.

Medical Equipment Rental & Sales – Terms & Conditions

By availing the rental or purchase of medical equipment from Adore Home Care Nursing Services (“Adore”), the Customer agrees to the following terms and conditions:

1. Advance Payment

  • All equipment rentals and sales require 100% advance payment at the time of booking. Equipment will not be dispatched or reserved until full payment is received.

2. Shipping & Delivery Charges

  • All shipping, delivery, and installation charges are to be borne by the Customer.

  • Delivery timelines will be communicated at the time of order confirmation and may vary based on location and product availability.

  •  Adore is not responsible for any delays caused by third-party logistics partners or unforeseen events.

3. Rental Period and Usage Policy

  • Rental charges are calculated monthly or as per the agreed duration at the time of booking.

  • Equipment is to be used only for the intended medical purpose and handled with care.

  • Any damage, misuse, or unauthorized modification to the equipment during the rental period will result in penalties or repair/replacement charges.

4. Return & Cancellation Policy

  • There is no refund if the Customer decides to return the rented equipment in the middle of the month or agreed rental period.

  • If early return is requested, the Customer is still liable to pay the full rental amount for that month.

  •  For product sales, returns are not accepted once the equipment is delivered and handed over in working condition, unless there is a manufacturing defect reported immediately upon delivery.

5. Equipment Condition

  •  All equipment will be delivered in clean and functional condition.
  • For rentals, a basic demo or usage guidance will be provided.
  • Customers are responsible for maintaining hygiene and cleanliness of the equipment during the rental period.

6. Damages and Liabilities

  • Any damage or loss to the equipment during the rental period due to mishandling, negligence, or misuse will be assessed, and the cost of repair or replacement will be charged to the Customer.

  • Adore is not liable for any injury or health complications arising from improper usage of equipment by untrained individuals.

7. Ownership

  • For rental equipment, ownership remains with Adore Home Care Nursing Services at all times.

  • The Customer must return the equipment upon completion of the rental period in the same condition as received (excluding normal wear and tear).

8. Product Warranty (Sales Only)

  • Products sold by Adore may carry manufacturer warranty as applicable. Warranty terms vary by product and brand.

  • Adore will assist in facilitating warranty claims, but is not responsible for the final decision by the manufacturer.

9. Amendments to Terms

  • Adore reserves the right to update or revise these terms and conditions at any time. Updated terms will be communicated via email or official platforms
